Fort Worth ISD will be having their annual FREE athletic physical examinations for any current 6th grade through 11th grade Fort Worth ISD student interested in playing sports during the 2018-2019 school year on Saturday May 5th at Wilkerson-Greines Activity Center. Times will be determined at a future date pending the numbers each school has participating.
All students who are wanting to participate MUST ride the bus from their school, not drop offs will be allowed. To get on the bus, at students MUST have a completed the medical history portion of the physical to include parent/guardian signatures. It will be of some assistance to have completed a portion of the vitals with the athletic trainer or coaches at their home school.
Students should wear athletic gear such as shorts, t-shirts, and athletic shoes. Students who wear contact lenses or glasses should have those with them on May 5th. Female students are asked to wear a sports bra or tank top under their t-shirt. DO NOT wear caps, jewelry, flip flops or sandals. Cell phones should be held by coaches as students go through the physical examination.
At Trimble Tech, it is encouraged for coaches to bring down those athletes who are wanting to participate during the athletic period at which time height, weight, and vision will be checked. Students will be then told to take the form home to complete the medical history portion with their parent(s)/guardian(s) and have them sign it. They are to then return it to the training room and hand it to the athletic trainer. If there is not parent signature on the medical history they will not be allowed go through the process on May 5th.
