With the 2018 school year right around the corner, FWISD is trying to make the process very easy so you can have all your paperwork completed before even setting foot on campus in August. Parents can now go to the internet and go to: https://www.rankonesport.com/content/
and the following screen should pop up:
In the top right corner of this screen, you see the portion that has “Fill out Electronic Forms” you will need to click on that which thing brings up:
You will need to select “TX” for the state of Texas, after this another screen will pop as pictures above and you will need to click on “Fort Worth ISD”. By doing so it will bring up the Rankonesport Fort Worth ISD page where you can download and print or what we prefer, filling out the Electronic Participation Forms. On the bottom of this page, click “Start Online Forms” in the blue box on the bottom of the page.
By clicking “Start Online Forms” another page will pop up giving you several options. You can create a new account, sign into an old account, or search for an old account. If you so choose to move on without an account the page above will pop up explaining the benefits of having an account. You can either continue with or without a account. Not having an account will not change the paperwork that you fill out. From here you will have to forms you will have to complete and they will look like this:
The first page is the FWISD Student – Athlete Medical Emergency Card, this has a lot of important information so please take your time filling this out in giving us the information we need to help your student should the situation arise that we need to. The coaches will have a copy of this or will be able to look this up when they travel, it also has the consent to treat form should they need to go to a hospital when they are not here at their home school. It is very important they fill out the top part with their first name, last name, Student ID, and the school they will be attending for the 2018-2019 school year. Once you fill out all the information both you and your student will need to sign so you will need a touch screen and/or a mouse. Click on the “I Agree” box and after both of you have completed that you must hit “Submit” in the grey box. If you choose to, you can enter your email or your students email and you/they will get an email notification the form has been approved. The second page is the “Insurance & UIL Signature Page” which has six different little boxes nest to links with different information. You and your student are responsible for know this information when you check those boxes. Once you have read over all of those and checked off on all six boxes, same as the first page once you fill out all the information both you and your student will need to sign so you will need a touch screen and/or a mouse. Click on the “I Agree” box and after both of you have completed that you must hit “Submit” in the grey box. If you choose to, you can enter your email or your students email and you/they will get an email notification the form has been approved. With that, you have completed the paperwork portion of having your student ready to participate in athletics for the 2018-2019 school year.
They still must have a current UIL Physical on a yearly basis as well as an up to date CVS (Concussion testing tool cognitive test) score on file. You can see the coaches or the athletic trainer at your students school for the most up to date physical form or you can find one at the following link:
