Everyone please mark your calendars now for Saturday, May 2nd, 2020 as that is when FWISD will be hosting their FREE MASS PHYSICALS at WGAC. This year more than ever this is really important, only physicals dated after May 1st, 2020 will be accepted for the 2020-2021 school year. All physicals will expire at the end of this school year no matter the date of the exam, unless it is dated after May 1st, 2020.

UIL Physical & Medical History Form

High school student-athletes turn in completed forms to the athletic trainer at your campus. Middle school student-athletes turn in completed forms to your middle school coach.  All Fort Worth ISD athletes entering the 7th – 12th grade must have an ANNUAL physical examination. The physical must be dated after May 1, 2020 to be accepted for the 2020-2021 school year. All physicals will expire at the end of each school year no matter the date of exam.

ALL physical forms must be dated in the top right corner with 2020 and have the section under question #21 on the medical history about the option to have an ECG performed if parents so choose to do so.

The link to this form, which will be available from the athletic trainers once they are printed for the mass physicals, can be found at the following link: Athletic and Marching Band PPE Form 2020-2021

Again this is very important as this is a NEW RULE for PHYSICALS for the 2020-2021 School year. This includes and is very important for cheerleaders who make the squad for next year. They are HIGHLY encouraged to go to mass physicals so they do not become ineligible after May 1st. More information will be coming out as we get closer to May, until then mark you calendars for May 1st and the free physicals held at WGAC by FWISD.
